α-mangostin exhibits both fast-binding and slow-binding inhibitions to FAS in vitro. It inhibits FAS overall reaction with a half-inhibitory concentration (IC50) value of 5.54 μM non-competitively with respect to NADPH, and partially competitively against both substrates acetyl-CoA and malonyl-CoA. α-Mangostin could reduce cell viability and induce apoptosis in human breast cancer cells. Alpha-mangostin significantly inhibits light-induced degeneration of photoreceptors and 200 μM H2O2-induced apoptosis of RPE cells. Alpha-mangostin can activate the expression of PKC-δ and down-regulate the expression of mitogen-activated protein kinases (MAPKs), including ERK1/2, JNK, P38.

Alpha-Mangostin stimulation results in an increase of superoxide dismutase (SOD) activity, glutathione peroxidase (GPX) activity and glutathione (GSH) content both in vivo and vitro. It suppresses light-induced retina dysfunction. Alpha-Mangostin has protective effect on kidney, heart and brain tissues against oxidative stress.
